Perhaps you're missing just 1 chest with the life-stone shard you need to get them all? There's a video from a youtuber that did walkthrough's for both Darksiders which mentions the chest in question and that's where I heard the guy saying that back then it was rumored that the "secret chest" was an error. Later I read on a gaming site I can't remember which now, sorry that the chest was in fact a bug but it didn't had anything needed to complete the game.

In the new game I can't be sure, but in the 1st game I didn't get that chest and I got all achievements.
